OBRC Report Form

The Ontario Bird Records Committee (OBRC) accepts rare bird reports in any written form the reporter finds convenient. This Report Form is intended to be easy to complete.

NOTE: the OBRC only requests documentation for species on the southern or northern Ontario Review Lists or for species and subspecies not on the Ontario Checklist. Before submitting your report, please consult the review list for Southern Ontario or for Northern Ontario or the Recognizable Forms Review List to ensure that the species you're reporting is on the applicable list.

We encourage you to provide as much detail as possible for each section below, especially Section 2 “Description” and Section 3 “How were similar species eliminated?” Please leave blank any details not observed or unknown. Drawings and photographs should be mailed separately to:
